These days, the barrage of news coming from Washington DC includes a lot that's being leaked to the media via anonymous sources. President Donald Trump and a number of lawmakers are saying the leaks are not just dangerous - they're illegal.

And now, there's been an arrest.

Insurance giant Humana Inc., which operates some of the nation's largest private Medicare health plans, knew for years of billing fraud at some South Florida clinics but did little to curb the practice even though it could harm patients, a doctor alleges in a newly unsealed whistleblower lawsuit.

The Justice Department said Friday that it had reached settlements totaling more than $250 million with hundreds of hospitals where doctors implanted cardiac devices in violation of Medicare coverage requirements.

There is a good chance that your once-independent doctor is now employed by a hospital. Dr. Michael Reilly, a Fort Lauderdale, Fla., orthopedic surgeon, does not believe this is good for physicians, patients or society.

For years he watched Broward Health, a nonprofit Florida hospital system, hire community doctors, pay them millions and minutely track the revenue they generated from admissions, procedures and tests.

One of the top Medicare billers in the country, Central Florida cardiologist Asad Qamar, is the target of two lawsuits accusing him of systematic Medicare fraud, including padding bills and performing unnecessary procedures.

WellCare Health Plans, which paid more than $400 million to settle past accusations of health-care fraud, is once again being sued under the False Claims Act.

The whistleblowers in this case are a half-dozen former WellCare staff members, including the former Vice President of Care Management Dr. Kirk Cianciolo. All are represented by Tampa attorney Kevin Darken, from the same firm that handled the previous whistleblower case against the Tampa-based WellCare.

All Children's Hospital has agreed to pay $7 million to settle a whistle-blower lawsuit alleging it violated anti-kickback laws by paying inflated salaries and bonuses to doctors so that they would bring in more patients and revenues.

Biogenesis, an "anti-aging" clinic in Coral Gables that injected performance-enhancing drugs into professional athletes, had well over 100 of them as clients going back to 2009 and at least a dozen who are still playing professional ball, the whistleblower in the case told ESPN.com
